Case Manager for Transition Aged Youth - RRH

Program Description:

Transitioning to adulthood is hard for anyone, but can be especially challenging for our most vulnerable experiencing homelessness. The TLC Rapid Re-Housing (RRH) Program provides housing and comprehensive supportive mobile case management for youth, 18-25, experiencing homelessness. Referrals are accessed through Sonoma County’s Coordinated Entry Database (CE) and are enrolled in our program through the Homeless Management Information System (HMIS). TLC partners with landlords and homeowners throughout Sonoma County to provide safe housing for our youth to live, while operating within a Housing First paradigm. Services are focused on assessing and addressing a young adult’s specific barriers to accessing housing (i.e. substance use, mental health symptoms, criminal background, finances) with the end goal of supporting them in obtaining long term affordable stable housing. Participants receive support from our trauma-informed dedicated staff within all areas of independent living including employment, education, finance management, accessing resources, and navigating the ins-and-outs of learning to live independently.

Position Summary:

TLC Child and Family Services is seeking a full-time (40 hrs/week) Case Manager for our Rapid Rehousing Program. Under the supervision of the Program Director, and reportable to the Chief Executive Officer of TLC, it is the responsibility of the Case Manager to:

  • Contact potential participants from the CE By Names Waitlist to determine their interest in our services. – Conduct intake interviews to identify youth’s housing needs, and interest in the program, while adhering to Housing First principles.
  • Coordinate with the Housing Director, Housing Navigator, and Program Director to link youth housing needs to TLC’s available housing.
  • Complete all mandatory HMIS documentation for youth in the program, including an initial intake, a yearly update (as needed), quarterly reports, and an exit assessment.
  • Complete monthly case notes.
  • Attend training relevant to homeless system services and or offered by agencies providing our funding for this program.
  • Represent TLC, specifically related to this program, in community meetings or collaborative efforts.
  • Knowledge of Rapid Rehousing and Various Voucher eligibility and guidelines.

Case Managers have a caseload of no more than 15 and are expected to engage in weekly contact and support, including at least once a month in the home. This position requires teamwork, dedication, commitment and patience in developing effective treatment plans for program participants, while implementing state, county and agency policies and procedures. This position requires flexibility with personal scheduling and meeting a variety of needs as assigned on a daily basis by the immediate supervisor. Some on call, night and weekend hours needed.

Essential Functions:

  • Conduct assessments of client strengths and needs and advocate for them accordingly
  • Aid each participant as they develop and meet the goals established in their Initial Assessment, outlining personal, financial, educational, employment and housing objectives
  • Maintain case records and provide timely reports on an individual participant’s progress and outcomes
  • Facilitate admissions through the CE By Names Waitlist and work collaboratively with the Housing Director and Program Director to identify available housing options
  • Complete Quarterly Reports, HMIS Assessments and periodic progress review reports and discharge summaries
  • Support participants with sustaining their housing by assisting them with following lease expectations including timely rent payment and property upkeep.
  • Monitor participant’s participation in employment, education, and independent living skills training
  • Ensure participant’s compliance with program rules and responsibilities
  • Monitor housekeeping and daily living skills
  • Support problem solving, conflict resolution, and planning counseling
  • Maintain appropriate working relationships with private and public agencies for the purposes of effective case management and collaboration
  • Participate in appropriate agency, state, and organization meetings as assigned
  • Document all contacts as appropriate in accordance with agency policy and good case management practice
  • Motivate and maintain effective working relationships with staff and community partners
  • Perform individual assessments and carry out appropriate interventions according to case management practice
  • Assist participants with accessing community resources including housing vouchers.

Abilities:

  • Demonstrate capacity to establish rapport, relate to the needs, and motivate youth and young adults.
  • Passion for working with a diverse group of transition age youth including LGBTQI youth, youth with disabilities, and youth from a variety of ethnic and cultural backgrounds
  • Excellent interpersonal skills (verbal and written) with proven ability to facilitate and connect youth and young adults to a broad variety of service providers
  • Implement Housing First Principles and willingness to support youth in addressing their housing barriers
  • Have an understanding on the effects of complex trauma and relationship to mental health, substance abuse and homelessness
  • Knowledge of independent living skills including education, employment and housing needs when working with transition age youth
  • A strength-based perspective is essential.
  • Ability to work both collaboratively and independently ~ Generally creative, optimistic, and flexible
  • Stay grounded and make decisions in occasional crisis situations
  • Possess good computer and word processing skills

Education/Training/Experience:

  • Bachelor Degree in social or behavioral health services plus one year experience with transition age youth/ young adults, homeless services, residential treatment, transitional housing, or foster care *or* 30 units in a social or behavioral health service field plus 4 years experience.
  • Applicants will be evaluated first on their experience in youth and family work and secondarily in experience in any related field
  • Bilingual in Spanish preferred
  • Must be able to pass a criminal history background check
  • Must be able to drive and transport the youth in employee’s personal vehicle and have a clean driving record and valid CA driver license
  • All employees will be required to familiarize themselves with a variety of written material that coincides with TLC Child and Family Services philosophic orientation toward youth and their treatment needs

Wage range: $23-$25.89/hour, depending on education and experience. Full-time positions include health benefits, paid time off, and a discretionary retirement program.
